APPLICATIONS WILL ONLY BE PROCEED AFTER RENEWAL OF FULL CLUB MEMBERSHIP.

Up to three tests can be submitted on this form. Forms maybe submitted between June 1, 2025 and July 15, 2025. No applications accepted after July 15, 2025, NO EXCEPTIONS.
ONLY Tests taken at an Oregon Skating Council Test Session are eligible for reimbursement.
DO NOT combine more than one test on a line.
You MUST have accurate test cost amount. Please refer to test schedule on the Testing page to look up test fee.

You may apply for reimbursement for up to three tests with this form. You may submit additional applications if needed. The CFSC Test Reimbursement Fund only guarantees reimbursement of 50% of two tests passed per year. If more funds are available a higher amount may be awarded.
