Test Reimbursement Fund Application 2025/26

for tests passed between july 1, 2024 and June 30, 2025

APPLICATIONS WILL ONLY BE PROCEED AFTER RENEWAL OF FULL CLUB MEMBERSHIP.

  • Up to three tests can be submitted on this form. Forms maybe submitted between June 1, 2026 and July 15, 2026. No applications accepted after July 15, 2026, NO EXCEPTIONS.

  • ONLY Tests taken at an Oregon Skating Council Test Session are eligible for reimbursement.

  • DO NOT combine more than one test on a line.

  • You MUST have accurate test cost amount. Please refer to test schedule on the Testing page to look up test fee.
